Position Description

Temporary Assignment - Approximately 52 Weeks

Position Summary
The Primary Care Nurse (PCN) is responsible for the coordination of care for patients donating or receiving kidney transplants, including ensuring all donor and recipient tests/procedures are completed for safe kidney donation/receipt. The PCN will coordinate communication and act as a liaison between multiple departments and services both at St. Joseph’s Healthcare and other hospitals in city and LHIN in addition to ensuring all policies and procedures carried out in accordance with St. Joseph’s Healthcare, Trillium Gift of Life and Health Canada.

Qualifications:

  • Current registration with the College of Nurses of Ontario required
  • Current BCLS/HCP certification required
  • CNeph certification required
  • Minimum of 2 years of Transplant nursing experiencing required, nurses with current Transplant experience will be given preference
  • RNAO, CNA or other professional association membership preferred

Demonstrated Knowledge, Skills and Abilities:

  • Advanced organization and coordination skills with the ability to prioritize workload to meet urgent and rapidly changing timelines.
  • Self-directed with strong leadership, problem solving, critical thinking, team building and interpersonal skills
  • Demonstrated proficiency with computers (e.g., Microsoft Office [Word, Outlook, Excel, etc.], internet browser searches, data entry and patient information systems)
  • Demonstrated ability to communicate (written and verbal) effectively with patients/clients and families
  • Demonstrated ability to practice collaboratively and communicate clearly with members of the interdisciplinary team to provide care for a full range of patients/clients
  • Demonstrated commitment to ongoing learning, professional development and continuous quality improvement
  • Excellent attendance record is required.
  • A resume regarding further education, special courses and experience should accompany the application
