To support us on our journey, Sinai Health is looking for a The Group Prenatal Care Coordinator - Registered Midwife to support our Family Medicine Team.

The Family Health Team provides high quality evidence-based care to patients in an environment that supports the education of learners and the evaluation and research of primary care practices and outcomes.

The Group Prenatal Care Coordinator – Registered Midwife will provide primary prenatal, antepartum, and postpartum care to low and moderate risk women in a busy, interdisciplinary, collaborative practice. The Group Prenatal Care Coordinator – Registered Midwife will collaborate with the family medicine physicians and residents, allied health team members (including social workers, registered nurses, registered dietitians, physical therapists, nurse practitioners, pharmacists) and other providers in the Sinai Health System and community to create and implement the highest standard of care.

Duties may be performed at the centre through appointments and group-care and in the community through off-site group sessions and home visits to contribute to the delivery of care for all pregnant patients of MSAFHT.

Responsibilities:

  • Coordinates group care for all of the pregnant patients who are part of the group prenatal care program, ensuring that programming is women-centred and meets the needs of the participants
  • Educates women and their partners about pregnancy, birth and the immediate first 6-weeks post-partum, as well as participates in well-baby care
  • Plans, implements, monitors, evaluates adjusts and documents outcomes of decisions and interventions and changes, as necessary, to optimize patient care.
  • Identifies the need for and facilitates follow up with members of the MSAFHT or referrals to specialists who are part of the Mount Sinai hospital-based obstetrical team or to community providers
  • Acts as the primary care provider for women and infants during post-partum home visits
  • Conducts a comprehensive clinical assessment, appropriate teaching and any needed interventions as well as timely referrals if required
  • Analyzes data and clinical outcomes to identify patient issues and trends in health care to inform decision-making
  • Develops new programs, including development of clinical practice protocols, group program curriculum and program evaluation plan
  • Advocates on the behalf of patients and their families, and other health team members to improve and enhance care
  • Collaborates with the interprofessional team in planning and evaluation patient care resources within the Academic Family Health Team
  • Participates and develops educational programs and initiatives based on the identified learning needs of the physicians and members of the allied health team to improve practice and deliver safe care
Job Requirements
  • Current registration in good standing with College of Midwives of Ontario with no conditions or restrictions on practice.
  • Certified lactation consultant or significant experience preferred
  • Minimum 5 years related clinical experience in primary maternity care
  • Sound clinical judgment and strong health assessment and diagnostic skills
  • Experience teaching family medicine residents and facilitating group education sessions. Demonstrated ability to work effectively independently and as a member of a multi-disciplinary team
  • Ability to work within a community-based context, with a culturally diverse community
  • Experience with and willingness to teach /work with other professional disciplines and students
  • Demonstrated understanding of, and commitment to, family centred care and evidence based practice
  • Demonstrated program development and program evaluation skills
  • Demonstrated commitment to professional growth and development
  • Demonstrated leadership and effective interpersonal skills
  • Demonstrated active participation in knowledge transfer
  • Experience providing community-based care
  • Ability to assist in the development and evaluation of protocols and procedures in order to provide optimal standardized care to patients
  • Basic computer knowledge including e-mail and the use of an electronic medical record
  • Experience teaching health professional learners in the clinical setting
  • Demonstrated satisfactory work performance and attendance history
