Niagara Health is embarking on a new quality improvement collaborative to connect primary care providers with community care, diagnostic imaging, and specialty healthcare services to support timely access to healthcare resources in Niagara and avoid unnecessary visits to the Emergency Departments and hospital re-admissions. The new program is called SCOPE Niagara- Seamless Care Optimizing the Patient Experience. Central to the success of this virtual collaborative is the SCOPE Registered Nurse (RN).

Reporting to the Clinical Manager Professional Practice, the successful applicant provides system navigation in collaboration with members of the interprofessional SCOPE team.

Responsibilities include but are not limited to:
o Serving as a point of access to connect family physicians to appropriate community, clinical and specialized supports to address patient health concerns
o Respond to all calls and emails made to the Scope Navigation Hub by the Primary Care Providers (PCPs) and their administrative assistants
o Triaging and forwarding referrals to appropriate SCOPE contact
o Navigating hospital programs, services, communication and resources that will assist PCP with managing client medical, functional, cognitive, psychosocial conditions
o Collaborating with the healthcare team to improve patient care as they transition through the healthcare system
o Enhance workflow and communication pathways between hospital, primary care and community care by identifying barriers to care and work towards creating new care pathways to promote and optimize seamless care for the client. This includes the clarification of referral criteria and identification of the resource that will best suit the healthcare needs

QUALIFICATIONS:
o Current registration, in good standing, with the College of Nurses of Ontario as a Registered Nurse (RN)
o Minimum of 5 years RN experience within acute care or post-acute care in the Niagara region, within the past 3 years
o Recent direct experience with patient care coordination in partnership with internal and external partners
o Strong understanding of Niagara Health and community healthcare resources and systems
o Experience working within an interprofessional team environment as well as independent and autonomous practice
o Recent experience triaging patient referrals
o Demonstrated assessment skills and ability to develop plan of care to address complex clinical situations
o Excellent communication skills with the ability to relay complex information effectively to patients, families, and health care professionals.
o Previous experience acting as a point of access contact with PCPs
o Superior inter-personal and leadership skills including critical thinking, responsiveness, conflict resolution, visionary thinking, organizational skills, effective customer service, decision-making skills and problem solving.
o Experience with leading the development and review of research and of evidenced based best practice pathways, protocols and guidelines in the care of people with complex health care needs
o Committed to excellence in all elements of professional practice, including excellence in patient care processes, measuring outcomes, collaboration and professional growth through continuing education and research utilization.
o Demonstrated ability to be flexible, self-directed and to work independently and as a member of the Professional Practice team
o Computer literacy required; database management skills preferred
o Consistent and regular attendance required.
o Vaccines (COVID-19 and others) are a new hire requirement of the job unless you have an exemption based on medical or on a ground pursuant to the Ontario Human Rights Code.
