Job Description
FIELD HOURS The Assessor Coordinator will be accountable for providing access and case management services for clients/patients within SHA, including contract tracing for COVID 19. This role will assist the COVID Case investigator by providing initial notification to exposed persons (contacts) and ongoing monitoring and assessment of contacts. The Coordinator provides information and facilitates access to a variety of resources, internal and external to SHA. The Coordinator acts as a liaison and advocate for clients/patients ensuring that those clients with the greatest need have access to service in a timely manner.
Job Qualifications
Required Qualifications
- Valid Class 5 driver’s license
- Current licensure with applicable provincial licensing body
- Bachelor Degree/Diploma in Nursing, or Bachelor Degree in Occupational Therapy, Physical Therapy, Speech Language Pathology, Social Work, or other health related field
- Effective oral and written communication skills
- Ability to work independently to plan and organize work load demands
- Knowledge of a Client Centered Case Management Approach
- Ability to work with a diverse group of clients
- Ability to problem solve and make decisions in a timely manner
- Demonstrated skill in advocacy, negotiation and conflict resolution
- Word processing skills necessary
- Ability to interact with or work in a team setting
- Demonstrated knowledge of community resources, internal and external to SHA
- Knowledge of/or willingness to become familiar with medical terminology
