The purpose of the Social Worker (SW), at Mount Hope Centre for Long-Term Care is to support and engage with residents, families, and essential care partners in the process of transition between the community and Mount Hope Centre for Long Term Care. To provide social work services, including but not limited to, intervention and support as necessary from admission through death/discharge. The Social Worker is a critical member of the interdisciplinary team to ensure the residents achieve optimal psychosocial and social functioning within the long-term care home.

As a Social Worker, you will demonstrate a resident and family centered approach. You will provide a full range of clinical social work interventions as appropriate to the needs of the residents, families and essential care partners, including psychosocial assessment, supportive counseling, and psychotherapy, consultation and education within an intergenerational context. You will also play a role in supporting residents, families and care partners with accessing services as required in the community. The social worker will play a vital role in the essential care partner program at Mount Hope. You will also support in guiding the practice of the social service workers. Lastly, meeting all standards and guidelines outlined in the Fixing Long-Term Care Act, 2021 and regulations 246/22.


Essential Qualifications

  • Minimum Bachelors of Social Work
  • Valid registration with the Ontario College of Social Workers and Social Service Workers as a Registered Social Worker
  • Minimum 3 years experience in social work practice in long-term care and community support service field is essential.
  • A police record check completed within the last six (6) months from a Canadian Police Information Centre (CPIC), inclusive of vulnerable sector screening, will be required of the successful candidate.
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Demonstrated competence in counseling and psychotherapeutic modalities with residents, families and essential care partners; including the areas of grief, bereavement, depression and mental illness, intergenerational issues, caregiver stress, adjustment to illness, hospitalization and long-term care
  • Demonstrated experience with placement issues (admission, transfer, family concerns, resident adjustment, discharge planning and counselling)
  • Facilitating and leading group work
  • General knowledge of the Public Guardian and Trustee Act, Powers of Attorney and Advanced Care directives.
  • Knowledge of relevant legislation including the Health Care Consent Act and Substitute Decisions Act
  • Excellent organizational, interpersonal and communication skills
  • Strong problem-solving ability and experience working with all levels of the organization
  • Comprehensive knowledge of and ability to mobilize community resources
  • Demonstrated strength in written/verbal communication and case management
  • Strong crisis intervention, advocacy, conflict management and facilitation skills
  • Highly developed interpersonal and problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Demonstrated skills in providing education to patients, families and the health care team
  • Proficient computer skills with a variety of programs
  • Ability to accept and positively influence change in the work environment
  • Familiarity with outcome measurement related to clinical practice, ability to measure and evaluate practice change outcomes
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications
  • Proficiency in French would be an asset
  • Master’s Degree in Social Work
  • Experience working with residents, families and essential care partners
  • Membership with the Ontario Association of Social Workers

Immunization Requirements
  • Provide documentation you have received three doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph’s Health Care London policies and processes related to Covid management in the workplace.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
