Shift:Mon: 1pm-5pm, Tues-Fri: 8am-5pm, Sat/Sun: Pharmacy is closed

SUMMARY:

This position will be based at the Cancer Centre Retail Pharmacy which is located on the 4th floor of the Grand River Regional Cancer Centre (GRRCC). This facility is situated directly adjacent to Grand River Hospital. The Pharmacist occupying this role plays an integral role in completing the circle of care at the GRRCC. There are various additional responsibilities that go beyond the traditional role of a Pharmacist in a retail pharmacy. The most prominent would be:

• Understanding the oncology regimens to provide a more comprehensive patient counselling experience
• Working closely with our Medication Reimbursement Specialist (MRS) staff to ensure patients have the necessary support to explore funding options for their treatment
• Conducting routine patient callbacks through our existing process to support patients on oral oncology medications
• Documenting these callback discussions in an easily retrievable format for clinical staff to view on the hospital database
• A very collaborative working environment with our clinical teams in the GRRCC to support a high level of patient care
• Timeous communication with prescribers for any urgent issues
• Maintain a detailed record of patient files on an ongoing basis
• Directly supervise each prescription fill and ensure correctness of all aspects of the prescription prior to the medication leaving the pharmacy

The Cancer Centre Retail Pharmacy is owned by Grand River Hospital. The position offers a unique professional opportunity to work with a multi-disciplinary health care team in the provision of pharmaceutical care to our patients. Patient safety is one of our primary objectives. The CCRP embraces the enhanced scope of practice for Pharmacists and is actively integrating regulated pharmacy technicians into daily practice.

QUALIFICATIONS REQUIRED:

• Bachelor degree in Pharmacy or PharmD

QUALIFICATION PREFERRED:

• The Pharmacist must have a current license with the Ontario College of Pharmacists, Part A of the Register with no history of disciplinary action on record • Pharmaclik Rx experience preferred – but not essential • 1-2 years of Hospital Oncology experience preferred
