Your Opportunity:Reporting to the Manager- Destination Home, the Occupational Therapist I - System Case Manager has a key role in facilitating positive discharge outcomes from acute care to the Edmonton Zone Home Care Program for those clients who have complex medical and psycho-social needs. The System Case Manager works collaboratively with Acute Care Transition Coordinators, community, and management teams within Continuing Care; identifies and provides recommendations on barriers to client flow to Home Care options. Utilizes (and assists with) information gathered from front line staff concerns, reports feedback to further identify system and individual barriers and provides leadership in system navigation. Major accountabilities also include but are not limited to quality and concerns management, practice development, collaboration, coordination and evaluation. The incumbent of this position will also be expected to work within various clients’ residences.Description:As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. Experience in a quality management, managerial or educational leadership role in continuing care or acute care with evidence of systems thinking within the broad perspectives of community & continuing care services. Knowledge of continuing care service delivery models, change management, quality improvement principles, computer technology & applications, conflict resolution & group facilitation processes. Knowledge & understanding of current & emerging health care issues. Strong commitment to customer satisfaction, quality health services delivery, education & research. Proven communication skills – clear concise & suited to target audience with proven strong negotiation, collaboration & facilitation skills. Proven ability to prioritize & organize workload & multiple priorities meeting tight deadlines. Understanding of acute & continuing care processes, goals & objectives. Ability to work in a fast-paced, changing health care environment. Resilience & flexibility; ability to work under pressure: strong work ethic.
