Pinegrove Place a 76 Bed faith based nonprofit care home in Richmond, BC is looking for a passionate social worker to provide comprehensive clinical and consultative social work services to our residents.

ROLE:

Reporting to the Director of Care, the Social Worker plans, organizes, and evaluates resources, services and programs pertaining to social services to ensure maximization of resident autonomy in decision-making and problem-solving. Working in partnership with the interdisciplinary team and community partners, is responsible for the admission and discharge of all residents. This person acts as an advocate for residents and families and ensures effective and efficient delivery of social work services during all phases of admission, transfer and discharge to or from Pinegrove Place. Facilitates the development of the Resident/Family Council. S/he collaborates with the other members of the Leadership Team to meet the goals and objectives of the organization in supporting the Mission, Vision and Strategic directions of Pinegrove Place.

QUALIFICATIONS:

1.EDUCATION

· Completion of a BSW from a recognized School of Social Work and current registration with the BC Colleget of Social Workers.

2.EXPERIENCE

· Minimum of 1 years’ experience the health care sector, preferably with experience with the geriatric population.

3.SKILLS and ABILITIES

  • A demonstrated ability to communicate effectively both orally and in writing.
  • Must demonstrate a range of interpersonal skills, including group, leadership, counseling and therapeutic positive interactions with residents, family and staff.
  • Ability to work within an interdisciplinary health care team whose members have diverse backgrounds and often competing priorities
  • Must be able to competently cope with external and internal demands related to this position.
  • Must demonstrate the ability to work under pressure and respond effectively in emergency situations.
  • Ability to anticipate and willingness to establish flex hours of work according to the assessment of care home requirements, in providing support.
  • Must have appropriate computer skills.

· Ability to organize work, set priorities and meet deadlines with a minimum of supervision.

RESPONSIBILITIES:

1 Develops, implements and monitors Social Work services for the residents by:

· Maintains the mission, vision and values of Pinegrove Place.

· Conducting psycho social assessments and evaluations for residents which may include social functioning, resident progress, financial concerns, family support and the need for ongoing counseling.

· Providing counseling to residents including but not limited to adjustment to facility living, clarification of concerns and ongoing planning; adjustment to chronic illness or disability; coping with loss due to terminal illness; family relationships and crisis intervention.

· Preparing a social history for inclusion in the resident’s health record.

· Consulting and communicating with external community agencies to access prescribed equipment for residents.

· Participating as a member of the interdisciplinary health care team.

· Developing and maintaining priorities for resident programs and appropriate referrals for rehabilitation therapy programs.

· Assessing residents’ psycho social needs in conjunction with the interdisciplinary health care team and documenting relevant information and participating in the team’s assessment, planning and evaluation of care plans to ensure the residents’ needs are being addressed.

· Organizing meetings with residents, family members and/or health representatives to facilitate problem solving and conflict resolution.

· Informing and gathering information for admission and care conferences.

· Referring to agencies and services which may improve the resident’s level of function, enhance quality of life and assist with financial concerns as needed.

· Collaborating with allied community services and government agencies to receive and interpret information on services, programs and policy changes which affect residents of Pinegrove Place.

· Developing and maintaining quality improvement indicators and evaluation of social work services including use of relevant records and statistics.

2. Acts as a liaison and provides services for families and community by:

· Facilitating the operation of the resident and/or family council as an advocacy and consultative meeting for families, and friends.

· Facilitating educational opportunities for residents’ families, and friends.

· Maintaining links with relevant community organizations and providing appropriate outreach support.

3. Acts as a resource to care home staff by:

· Planning and delivering in-service education sessions for other members of the health care team as required.

· Participating as a member of appropriate Pinegrove Place committees.
