Job Description


The Social Worker, in collaboration with the interprofessional Geriatric Outreach Team, provides community outreach and follow up care to community-dwelling seniors (including those in long-term care homes and various other supportive housing situations) who are 65 years of age and older. This position will report to the Clinical Services Manager, Senior’s Health, with oversight from the clinical director of the Senior’s program. The primary aim of this role, and overall service, is to maximize client functioning and promote community quality of life.

Accountabilities:

  • Providing direct social work services to clients and their families/significant others
  • Coordinating community care discharge planning
  • Providing services in the area of community care and social work services

Qualifications
  • Masters degree in social work required
  • OCSWSSW membership and in good standing
  • Minimum five years experience working with geriatric patients/residents
  • Current registration in good standing with the Ontario College of Social Workers and Social Service Workers
  • Minimum two years experience in a hospital setting preferred
  • Strong counseling skills in individual, family and group mediums
  • An understanding of psychosocial rehabilitation principles and sound knowledge of the theory and practice of professional social work
  • Basic understanding of standard assessment tools
  • Knowledgeable in the provision of professional social work services to clients and their families/significant others/care givers (such as individual, group, marital or family counseling, discharge panning etc.)
  • Excellent psychosocial assessment and intervention skills, including crisis intervention
  • Demonstrated understanding of geriatric and/or psychiatric theory and practice to include knowledge specific to the elderly and aging process
  • Possess community development skills and the ability to develop partnerships with other providers
  • Demonstrated understanding of geriatric syndromes, serious mental illness, and cognitive and physiologic impairments associated with dementia
  • Knowledge of community resources and ability to work with community partners
  • Demonstrated ability to effectively perform in an inter professional team environment
  • Demonstrated effective communication, team building, problem solving and conflict resolution skills
  • Demonstrated ability to engage and work collaboratively with all levels and members of the inter professional team
  • Demonstrated effective judgment and innovative change management skills
  • Demonstrated ability to utilize quality, safety and risk-based frameworks to utilize critical thinking, priority setting and problem-solving skills
  • Excellent written, verbal communication and documentation skills
  • Detail oriented with excellent organization and time management skills
  • Proven good attendance and work record
  • Will be required to travel between all Osler sites and the Ontario Health Team area; valid driver’s license and access to transportation required

Hours: Currently days, 8 hour shifts including weekends and statutory holidays (subject to change in accordance with operational requirements)
