We are seeking a dynamic, highly motivated Client Care Workerwho demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.

The Client Care Worker, within a multi-disciplinary team, actively provides support and therapeutic services within a residential setting to individuals experiencing homelessness and who have been diagnosed with COVID-19. Services are based on a residential harm reduction approach to service delivery. At times, position is mobile within the community and will be responsible for conducting outreach to support an understanding of COVID-19 safety measures and isolation protocols and processes. This position is a unionized, temporary, full-timeposition.


Key Responsibilities:


  • Collaboratively and efficiently work within a multi-disciplinary team of health and social service professionals.
  • Strictly adhere to COVID-19 isolation protocols while on site and in the community
  • Work effectively within a strengths based client-centered approach to service delivery
  • Assist with program engagements, assessments, case planning, and appropriate referrals of clients to facilitate client movement through the program and connection to community resources.
  • Administer medications and appropriately document distribution, according to the established guidelines of the program.
  • Deliver harm reduction supports, according to the established guidelines of the program and in collaboration with community partners.
  • Actively engage individual clients to assess their readiness, motivation and confidence to move through change, ensure residential stability and safety within the program.
  • Participate in meal planning (menus) consistent with nutritional guidelines, maintenance of food supplies and all other program supplies. Prepare and serve meals, keep dining room and kitchenette clean according to public health requirements.
  • Take part in regular cleaning of units and common areas to ensure strict adherence to COVID-19 infection control and pest management protocols
  • Document and input information, in individual client files and all program communication logs and information sheets, manually and in the Client Outcome Tracking System.
  • Respond according to program protocols to emergency or crisis situation.
  • Assist in the development, managing and delivery of programs and activities for clients.
  • Assist clients with personal hygiene, as necessary.
  • As needed, conduct outreach within the community and in the shelter system to support individuals within the homeless population to understand COVID-19 safety measures and follow isolation processes and protocols established with the City of Hamilton and other community partners.
  • On an as needed basis and based on availability, provide support to other teams throughout the organization working to support individuals living in a residential setting.
  • Attend and participate in all relevant staff meetings and training opportunities, as requested.
  • Provide coverage for other employees on shift, as required.
  • Other duties as assigned

What you will bring:


  • Post-secondary degree/diploma in Social Services, Developmental Services or relevant equivalent required
  • Concurrent Disorders, Mental Health and Disability Management or equivalent certificate is an asset
  • Minimum of 1-2 years of experience, effectively working with marginalized populations experiencing homelessness and a demonstrated ability to form rapport with target populations preferably in a drop-in or residential setting.
  • Must be comfortable working in a residential setting working directly with COVID-19 positive clients
  • Experience providing client-centered services within a multi-disciplinary team of providers.
  • Strong communication and documentation skills.
  • Must be willing to work flexible hours.
  • Training in non-violent crisis intervention (NVCI), ASSIST, first aid/CPR certification, P.I.E.C.E.S., Naloxone, first aid mental health or relevant equivalents is an asset; however, organization will provide Naloxone, NVCI, ASSIST and first aid mental health training for the successful candidate.

As a condition of employment, the successful candidate is required to submit proof of full COVID-19 vaccination to Human Resources. In the event that successful candidates are unable to get their COVID-19 vaccination, completion of an educational session and weekly antigen testing to provide a negative result will be required.
