Job Description

The Assessor Coordinator will be accountable for providing access and case management services for clients/patients within SHA, including contract tracing for COVID 19. This role will assist the COVID Case Investigator by providing initial notification to exposed persons (contacts) and ongoing monitoring and assessment of contacts. The Coordinator provides information and facilitates access to a variety of resources, internal and external to SHA. The Coordinator acts as a liaison and advocate for clients/patients ensuring that those clients with the greatest need have access to service in a timely manner. Qualifications: - A degree in nursing, social work, physical therapy or occupational therapy. - Current registration with the appropriate licensing body

224 hours of work in a 6 week Rotation.
