Shift:7.5 Hour Shifts Monday to Friday – Days (0800-1600)

SUMMARY:

Duties include the following but are not limited to: Incorporating the attitudes, knowledge and skills required for ensuring the high quality of comprehensive care for patients with Chronic Kidney Disease (CKD) & Nephrology disorders. Working within a leadership role that supports early management of CKD identification, prevention and the concepts of patient self care management. Patient screening, preparation and promotion of independent modalities (Home Hemodialysis & Peritoneal Dialysis). This is accomplished within a mentorship environment; in collaboration with the Nephrologists, the Allied Health team and the MCKC/PD clinic nurses. Demonstrated experience, ability and knowledge in the principles of adult teaching and care of predialysis, peritoneal & hemodialysis and pretransplant patients. The incumbent will work at both KW (primary site) and Guelph Renal Clinic site (as required). The candidate will contribute to ensuring there is a safe environment for patients, staff and visitors.

QUALIFICATIONS:

• Current Certificate of Competence – College of Nurses of Ontario in good standing, BSCN preferred.
• Current certification in Nephrology Nursing – Canadian Nurses Association
• Minimum of 5 years current clinical experience required in an outpatient Nephrology/MCKC setting.
• Current experience in Home therapies (PD and Home Hemodialysis).
• Experience in acute / chronic hemodialysis.
• Completed adult education course and applied experience in CKD management and ability to utilize the recognized principles of the Right Start framework for home dialysis therapies.
• BCLS
• Certification in Blood Bank administration, Renal Drug Therapy, Access Management
• Demonstrated knowledge of nephrology theory and clinical renal nursing practice
• Demonstrated skills in assessing, planning, implementing and evaluating renal patient education for the care of predialysis, pretransplant, and independent modalities of peritoneal & hemodialysis within a patient self care management environment.
• Demonstrated expertise in management of health problems and support needs for predialysis patients in either a home or clinic environment.
• Ability to collaborate effectively with members of a multidisciplinary team, patients and families, homecare services and other health care facilities.
• Demonstrated experience with Continuous Quality Improvement
• Must have valid drivers license
• Good attendance record
