Job Description

As an Independent Facilitator, will walk alongside children, youth, adults, and seniors with diverse identities, including disabilities, supported by their identified families to realize their desires and achieve their goals for the life they choose.

Facilitators are guided by values and principles that promote self-determination and expression, inclusion, social justice, citizenship, community participation and contribution. They are committed to the individual’s goals, creating a “good life” as defined by the person, building trusted relationships and social networks, active community inclusion, and assisting them in claiming greater control over their life. Facilitators use a person-directed approach and provide resources offering the person and family the freedom to make informed, quality decisions.

Responsibilities and Activities

  • Use various facilitation tools, practices, and resources that empower the person to envision and create the life they want.
  • Facilitate the development of a personalized “Embracing Possibilities Life Plan” with pathways to achieve their goals in alignment with their goals, unique gifts, strengths, and personal interests.
  • Conduct research guided by the need of the person and/or family.
  • Identify, access, explore and experience various non-segregated community-based activities and resources with

the person to determine fit and achieve their goals.

  • Walk alongside the person as a guide, resource, and ally as they develop their skills, capacity and support for greater independence and are empowered to achieve their goals.
  • Facilitate relationship-building and the development and maintenance of a network of support that offers safety, security, and sustainability of goals.
  • Apply an ecological approach to address the social determinants of health, including the person’s physical, psychological, social, spiritual, and economic health and well-being.
  • Participate in formal and informal training, small group, and individual mentoring,

Qualifications

· Training in or relevant to Independent Facilitation and Person Directed Planning (social justice, community

development, persons with disabilities, mental health, etc.)

· A passion for working with individuals in complex and dynamic situations.

· Commitment to the values and ideals of Independent Facilitation and Planning:

o Social inclusion and citizenship, self-determination, meaningful reciprocal relationships with others, and

personal security.

o Awareness of the nature of the challenges confronting individuals with developmental disabilities, including

institutionalization, inequities, intersectionality, power imbalances

  • Ability to take direction from the individuals you are walking alongside.

  • Flexibility and adaptability to complex, dynamic, and changing situations
  • Self-directed, organized, taking personal responsibility and accountability.

Requirements

· Flexible availability for both daytime and evening and, at times, weekend work.

· Strong knowledge of formal and informal resources and networks in Waterloo Region; knowledge of Cambridge

and/or the Townships is considered an asset.

· Availability to work throughout Waterloo Region.

· Access to reliable transportation.

· Willingness to submit a vulnerable sector police record check.
