Your Opportunity:

As a Single Independent Living Support Worker (ILS) you will be part of an interdisciplinary team, assisting clients to develop and maintain skills as identified in the treatment plan associated with living independently in the community. The ILS worker will be working under the direction of a health care professional (MHT, Nurse, Psychiatrist) with individuals in the community who are experiencing addictions and mental health problems. The ILS worker assists the MHT or designated health care professional with the implementation of the client’s treatment plan by providing goal oriented independently living supports. The ILS worker relays factual, timely information to the MHT which assists in the evaluation and refinement of the overall treatment plan.

As a Single Independent Living Support (ILS) worker, you will be part of an interprofessional team, assisting patients (client, resident) to develop and maintain skills as identified in the care plan, associated with living independently in the community. You will provide support with preventative health care, health maintenance, quality of life, life skills coaching and connect patients to appropriate community supports. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of Grade 12 or equivalent.

Additional Required Qualifications:

Two years’ experience working with persons with addiction and mental health concerns. BLS Level C. Nonviolent Crisis Intervention. Experience working in a community based setting.

Preferred Qualifications:

Social Work diploma or Mental health aide certificate. Equivalencies may be considered.
