Job Description
The Assessor Coordinator will be accountable for providing access and case management services for clients/patients within SHA, including contract tracing for COVID 19. This role will assist the COVID Case Investigator by providing initial notification to exposed persons (contacts) and ongoing monitoring and assessment of contacts. The Coordinator provides information and facilitates access to a variety of resources, internal and external to SHA. The Coordinator acts as a liaison and advocate for clients/patients ensuring that those clients with the greatest need have access to service in a timely manner.
Additional Information
Required Qualifications: A degree in nursing, social work, physical therapy or occupational therapy. Current registration with the appropriate licensing body Knowledge, Skills & Abilities: Knowledge of a client centered case management approach Ability to work with a diverse group of clients Ability to interact with or work in a team setting Able to work independently and plan and organize time efficiently Ability to problem solve and make decisions in a timely manner Effective communication skills Demonstrated skill in advocacy, negotiation and conflict resolution Demonstrated knowledge of community resources, internal and external to SHA Knowledge of/or willingness to become familiar with medical terminology Word processing skills necessary A current valid Saskatchewan drivers license and access to a vehicle in good repair is required. This is a regional position currently based in Meadow Lake Some travel may be required.
