This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. The Follow-up Support Worker at YMCA of Northern Alberta plans, implement and supports the execution of the Housing First Philosophy program to individuals, couples and families who are chronically homeless with multiple barriers and high needs. The Follow-up Support Worker is accountable to the YMCA team and participants as a whole to ensure successful program delivery.

ESSENTIAL FUNCTIONS:
1. Build relationships with staff, volunteers, and participants with regular communication and

timely responses to messages, calls and emails, follow-up and document any concerns as

needed. Interactive with everyone professionally and positively.

2. Be an active participant within the staff team to reach volunteer recruitment goals, work as a

role-model on staff and volunteer retention and recruitment.

3. Role model behaviors when working with participants and offer high quality services; ensure

that staff and volunteers are also delivering services matching YMCA Standards. Recommend

changes to enhance services.

4. Contribute to building the team by assisting in orientations and training and by providing

feedback and appreciation to staff and volunteers; ensure they are following all non-negotiables

and give them guidance on their performance. Discuss and document any concerns as needed.

5. Plan, implement and evaluate programs within YMCA guidelines and contracted performance

measures. Recommend quality improvement ideas and implement improved changes to

programs and services. Collect statistics and follow-up with participants and staff to ensure.

7. Communicate with participants and provide intensive case management support. Visit participants weekly to support them in maintaining their housing, improving life skills and help them build a connection to their community. Develop and monitor Individual Action Pland and work with participants to meet their goals. Give them appropriate resources as needed.

8. Effectively communicate the progress of participant within the team and to relevant third parties, as appropriate.

9. Identify and respond to critical situations, problem solve in consultation with the supervisor and implement agreed upon solutions.

10. Complete all administrative duties, including but not limited to; maintaining participant files, compiling monthly statistics, quarterly outcome reports; as needed accurately and in a timely manner.

11. Work within a budget, and maintain claims forms and tracking sheets. Communicate information to contribute to forecasting and budget activities.

12. Establish community relations relevant to the program, network with other providers of Housing First Services. Provide information about service to internal and external stakeholders.

13. Create and share success stories as appropriate.

14. Work as a flexible team member; taking on other duties throughout the facility as required and by participating in relevant community and promotional activities when necessary. Follow procedures when it comes to monitoring equipment for safety, and working as Manager on Duty.

15. Know and reviews all emergency procedures, follows all guidelines from Risk Management and Health & Safety including but not limited to; Work Alone Policy and Procedures and the Children, Youth and Vulnerable Adults Protection Policies and Procedures.

16. Consistently represent YMCA’s values and mission, comply with YMCA Policies, CHI Standards of Proactive, SAM Standards and funder mandates. Continuously act towards achieving both personal and YMCA goals.
