Registered Nurse (Casual, Part-time)

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR), has been awarded a WSIB contract to complete Independent Living Services (ILS) assessments throughout the province of Ontario.

We are currently hiring for Registered Nurses to complete Assessments throughout Ontario.

The Following Types Of Assessments Will Be Provided
  • Personal Care Assessments (est. 5-6 hours) assist in determining the level of assistance required with activities of daily living, if any, for a severely injured/ill Worker. The assessment will be conducted at the Worker’s home, basing it upon the WSIB report template and the Activities of Daily Living Scale (ADLS) form. Based on the information provided by the WSIB, the clinician should have a clear understanding of the Worker’s medical history and work-related injury/illness prior to initiating the assessment. This assessment may be conducted Registered Nurse and must have at least 3 years relevant experience.
  • Home Safety Assessments (est. 3-4 hours) are required to assess a Worker’s functional abilities within the home to identify any potential hazards, safety concerns and/or non-structural barriers (e.g. identifying fall risks, recommending safety devices such as bath grab bars) as it relates to the work related injury/illness. The assessment will recommend strategies and solutions to promote the Worker’s safe interaction within their home environment. The recommendations should reflect the likelihood of whether the Worker’s health care needs may change if their injury/illness progresses. This service may be conducted by a Registered Nurse with at least 3 years experience in the provision, coordination and implementation of various appropriate equipment and assistive devices for home.
