Have you had a recent medical test which required a clinic/ lab to provide a confirming the result?

Each image must be taken of a physical document which has come from a clinic/ lab and shows clearly the test that was undertaken and the result of that test.

Requirements

Must have a physical/paper copy of lab test result (No online results will be accepted)

You may submit more than one test result; assuming the form/ test is unique across submissions.
