L’équipe de santé familiale académique Montfort est un groupe dynamique, formé de médecins et autres professionnels de la santé. Notre vision est de fournir des soins d’excellence personnalisés dans un milieu de formation pour les professionnels de l’avenir.

Nous sommes à la recherche d’une personne indépendante et motivée afin d’occuper un poste de Pharmacien clinicien à temps partiel, soit 15 heures par semaine (2 jours).

RÉSUMÉ DES FONCTIONS :

Le pharmacien travail en collaboration avec l’équipe interprofessionnelle de santé familiale. Il vise principalement à optimiser la médicamentation chez les personnes âgées en révisant les cas de polypharmacie tout en éduquant les patients quant à la prise de leurs médicaments.

COMPÉTENCES EXIGÉES

• Capacité de communiquer couramment, verbalement et par écrit, en français et en anglais (condition absolue)
• Baccalauréat en pharmacie ou diplôme d’études supérieures en sciences pharmaceutiques (avec spécialisation en pharmacie clinique)
• Capacité de soigner et d’enseigner par l’entremise de moyens informatisés (ex. Zoom)
• Adhésion courante comme pharmacien l’Ordre des pharmaciens de l’Ontario.

ATOUTS

• Expérience en enseignement de groupe et/ou individuel ;
• Expérience clinique dans le domaine de la gériatrie.

SALAIRE:

Le titulaire sera rémunéré selon une échelle salariale à six (6) niveaux d’entre 40.68$/heur à 49.89$/heur.

The Montfort Academic Family Health Team is a dynamic group of physicians, nurses and other health professionals whose vision is to deliver personalized care excellence in a learning environment for future healthcare professionals.

The Montfort Academic Family Health Team is looking for an energetic and self-motivated person to fulfill the part time position of Clinical Pharmacist within its interdisciplinary health team. The position is offered at 15 hours per week.

POSITION SUMMARY:

The Pharmacist works in collaboration with the interdisciplinary primary care team. He or she works at optimizing medication use for our elderly patients by revising polypharmacy cases and by educating patients with respect to their medication use.

REQUIRED SKILLS

• Strong written and oral language skills in both official languages (French and English).
• Undergraduate or graduate degree in pharmaceutical sciences (specializing in clinical pharmacy)
• Ability to treat and to teach virtually (such as Zoom)
• Currently in good standing with the Ontario College of pharmacists.

ASSETS

• Experience in individual and/or group teaching;
• Clinical experience in at least one of the following fields: critical care, secondary prevention of cardiovascular disease, pulmonary disease or diabetes.

SALARY:

The incumbent will be remunerated as per a 6-level hourly salary scale ranging from $40.68 to $49.89.
