Director, Transitional Care, Patient & Client Care Administration

$118,365 - $139,249.50 Reference#:21-862A

The Director, Transitional Care will be responsible for the development, implementation, and execution of the

strategic vision, planning expertise, and mission focused program leadership required to achieve operating goals

for the assigned programs. The individual will determine the need for and priority of allocation of human,

material, and financial resources in order to promote the growth and expansion of these services and promote

an integrated Program that achieves the strategic and operational objectives and aligns with Providence Care’s
Mission, Vision and Values.

The Director will provide leadership in system coordination across the Southeast Region by developing

relationships across health sectors and will represent Providence Care in the community at a local and regional

level. The individual will be responsible to ensure the development, coordination, and capacity enhancement of

the system design and operational processes with an emphasis on patient/client and family centred care, quality

improvement, fiscal accountability, and timely access to care and transitions (right care at the right time at the

right place) related to Providence Care programs/services. To facilitate the system capacity enhancements, the

individual will engage and collaborate with sector specific partners and networks, Operational Directors and

Managers, and Clinical Directors.

Role Responsibilities

  • Promotes excellence in patient/client and family centred care and encourages the ongoing review of
programs and services against evidence-based practices, patient/client and family co-design and

feedback, emerging trends, ministry policy and program developments and profession specific practice

standards.

  • Develops and enhances partnerships with other programs and external community agencies in order to
continually improve service coordination, timely access to high level quality care, and an integrated

model of care

  • Utilizes a participatory and collaborative approach to guide inter-professional teams to develop and
achieve established goals and objectives for care and services, based on implementation of results-

based practice

  • Leads and supports the development and implementation of appropriate evaluation methodologies at
the level of the individual, team, program, and system to support the delivery and optimal outcomes of

patient/client and family centred care

  • Promotes and maintains safe care by proactively identifying and mitigating risks and promoting
adherence to all standards as required by Providence Care, provincial regulations, and Accreditation

Canada, best practices, and evidence-based standards of care


  • Provides leadership and develops goals and objectives for the delivery, planning and evaluation of

programs in collaboration with community partners, Operational Directors and Managers, Clinical

Directors, staff, and physicians

Required Qualifications and Experience:

  • Master’s Degree in a health-related profession or a combination of formal education, development and
work experience may be considered
  • Current unrestricted registration with an applicable Regulatory College
  • Five years of progressive experience in health care management with at least three years in clinical

operations management; experience in managing Rehabilitation and/or Complex Medical Management

Programs and/or Specialized Geriatrics Programs, and/or Mental Health Programs preferred

  • Experience in managing multiple hospital functions, preferred
