Summary:
Native Child and Family Services of Toronto strives to provide a life of quality, well-being, caring and healing for our children and families in the Toronto Indigenous Community. We do this by creating a service model that is culture-based and respects the values of Native people, the extended family, and the right to self-determination and through the integration of holistic, culturally based prevention services that are child centred, family focused, and community driven.
The Community of Care Circle (CCC) coordinator will work alongside Child and Family Wellbeing (CFWB) workers, and Holistic Services (HS) workers to provide coordination for families and/or Community members who work with more than one service at Native Child and Family Services of Toronto (NCFST). The purpose of the Community of Care Circle is to support interdisciplinary team members to deliver integrated, holistic, and trauma-informed services that are grounded in Indigenous values, knowledge, best practice, and worldview.
Primary Responsibilities:
Under the direction of the supervisor, Intake Team, the Community of Care Circle coordinator will:
  • Facilitate an understanding of the roles of each member in the Circle of Care.
  • Support with reviewing and coordinating Community Member treatments or service plans.
  • Identify any gaps or service duplication for the Community Member, or family.
  • Enable ongoing collaboration between the Circle of Care to better meet the needs of the family and/or the Community Member.
  • Ensure staff support Community by working within the interdisciplinary team, and support wrap-around care.
  • Educate staff (internal and external) on the Community of Care Circle, and its process.
  • Field referrals for Community of Care Circles (CCC’s).
  • Collaborate with staff to ensure timely and effective communication, for the benefit of Community Members.
  • Work alongside the Original Talking Circle (OTC) facilitator, and support in identifying potential families or Community Members who may benefit from the OTC.
  • Other duties as it pertains to this role.

Qualifications, Knowledge and Skills:
  • Strong understanding and practice of Indigenous approaches to healing, including land-based practices, cultural knowledge, ceremonies, and teachings.
  • Experience in social services, OR a combination of education, lived experience, and experience working with the Urban Indigenous population.
  • Strong facilitation skills in conducting traditional talking and sharing circles with staff and community.
  • Ability to hold compassionate, and safe space for staff (internal and external).
  • Excellent written and verbal communication skills; organizational skills; initiative and respect for confidentiality.
  • Ability to work effectively with all levels of staff to maintain effective communication and working relationships, demonstrating strong interpersonal skills, tact, sensitivity and building strong internal relationships.
  • Demonstrated understanding, and commitment to, integrating the Native Child and Family Services of Toronto Mission and values into practice, service, and relationships.
  • Demonstrated understanding of the workplace health and safety practices and understanding of an employee’s responsibility under current legislation.
  • Demonstrated understanding of the unique dynamics that exist within the First Nations, Inuit, and Métis cultures.
  • Strong understanding of the current Child and Family Wellbeing mandate and role, as well as an understanding of Holistic services as it related to their role within the Circle of Care.
